Health and Safety Considerations

Purpose- The purpose of this document, is to get signed permission. From anyone under the age of sixteen from a parent in order to have their photo taken for a photo shoot.

Why this document might be used within the creative industry-  This might be used in order to have documented proof of consented permission from the parent and child. So that they can not get arrested for uncontested images.


Why you might need to use this within assignment 3- You would possibly need this form within your assignment, in order to take the images for a stop motion video and to place images on a site for viewers purpose.

Purpose- The purpose of this image, is to have signed proof of consensual permission from anyone over the age of  eighteen years old. 

Why it might be used in the creative industry- This might be used to show signed proof of consent to let unprofessional photographers take photos of the models within a photo shoot. 

Why you might need to use this within assignment 3- You might need to use this if you was going to take images of people over the age of eighteen.  

Purpose- The purpose of this document, would be to have signed proof from a land owner. In order to shoot on their land and if anything was to happen within a certain time period. They would have written proof to either defend or clarify, it was not or was them.   

Why it might be used within the creative industry- It might be used to show proof they are not shooting illegally on private land and that if any incidents accrued within the time period they where not on the land they would have written proof to defend against them that it was not them. 

Why you might use this within assignment 3. - You might need to use this type of document, to have proof of permission to hold a photo shot on private land. 

Purpose- The purpose of a recce form is to make sure that a location is suitable for a shoot; it ensures that everything necessary for the shoot such as lighting and sounds is located on site.

Why it might be used within the creative industry- To save you time and money, and avoiding using a location that is not suitable or available to you without wasting time. 

Why you might use this within assignment 3. - I may want to use use a location for a photoshoot; this form would enable me to make sure I have all of the correct permissions, authorizations and information needed.

Purpose- The purpose of this document is to have proof that the operator the activity or task can identify and understand the risks. also that know how to handle the situation.

Why it might be used within the creative industry- It would be used to show that all people involved understand and can identify the risks. Also that if one accrued they would be able to resolve it and handle them.

Why I might use this within assignment 3-  I might use this to show understanding and identification. Of the risks involved, if I was to possibly to shoot in a risky area or a abandoned location.
